We are unable to initiate Physical Therapy without having the "Consent/Financial Agreement" and the "Patient History" forms signed and dated. It is very important that you print/fill them out prior to your first visit. Doing so allows us to begin our evaluation right away and makes the most efficient use of our time together.

If you are unable to fill the form out online, you can print it out and fill it out by hand. Please give it to your therapist at your first visit. Please note, the formatting/numbering of the questions was changed when converting to paper. Please fill it out as accurately and completely as possible.
